As we celebrate the milestone 20 years of serving our local community, we want to give back in a meaningful way.
To recognise the support we have received over the past two decades, Pond Square Dental is launching a Smile Transformation Community Initiative. As part of this initiative, individuals may apply themselves or nominate someone to be considered for a smile transformation.
This is not a prize draw or public vote. It is a discretionary, clinically led initiative designed to provide selected individuals with the opportunity to receive treatment, subject to assessment and suitability.
The Process
1. Submit an Application
Applicants must complete the official online application form. Applications submitted via email, social media, comment, or third party will not be considered.
2. Internal Review
All applications are reviewed internally by the Practice. Selection is based on clinical suitability, feasibility, complexity, commitment, and alignment with the nature of the initiative.
3. Clinical Assessment
Shortlisted applicants may be invited for a clinical assessment. This may include examination, photographs, radiographs, and diagnostic records.
Attending the assessment does not guarantee treatment.
4. Treatment Proposal
If selected, a written treatment proposal will be provided. Only treatment confirmed in writing forms part of the initiative.
Important Information Before Applying
- Applications are open to existing and new patients.
- Applicants under 18 must have parental or legal guardian consent and will be considered only where clinically appropriate.
- Participants must be able to attend appointments and commit to agreed timelines.
- The treatment journey will be documented as part of this initiative.
- Clinical treatment provided by the practice forms part of this initiative. A nominal fee may apply where laboratory work is required.
